Managing Life Stages, Transitions and Supports

As a registered support coordinator, Empowered Therapy and Training can provide short and long-term supports that focus on strengthening your ability to coordinate the supports and services that you need, whilst assisting you to live at home and participate in the community either individually or as part of a group.

Our Support Coordination services include:

1. Support connection
2. Coordination of supports
3. Assistance with accommodation and tenancy obligations
4. Life transition planning including mentoring, peer support and individual skill development
5. Assistance with decision making, daily planning, budgeting

Support Connection – Assistance for participants to implement their plan by strengthening the ability to connect with the broader systems of supports and understand the purpose of the funded supports and participate in the community.

Our Support Connection service will assist a participant to understand the aspects of the plan, assisting in the ongoing management of supports, and answer questions as they arise.

Coordination of Supports is a way of focusing on supporting participants to direct their lives, not just their services. This may include resolving points of crisis and developing resilience in the participant’s network.

Our experienced Support Coordinators work to strengthen a participant’s ability to design and build their supports with an emphasis by linking the broader systems of support across a complex service delivery environment.
